The handbook opens with the International Patient Safety Goals — identifying patients correctly, improving effective communication, improving the safety of high-alert medications, ensuring correct-site and correct-patient surgery, reducing healthcare-associated infections and reducing harm from falls — then works through twelve chapters covering why errors happen, systems and safety culture, communication, recognizing and responding to risks, infection prevention, safe medication practices, partnering with patients, teamwork, learning from errors and near misses, the nurse's role in building safe systems, and the digital, legal and ethical aspects of patient safety.
